Make all debug settings in notes.ini start with 'DEBUG_'

Most debug settings start with DEBUG*, but some are constructed without '_' or even with DEBUG in the end of the setting.
 
Consistency is not very obvious....
 
Examples:
1. SMTPClientDebug=1
2. IM_Request_Debug=17
3. HTTPEnableThreadDebug=1
4. AgentThreadDebug=1
5. inetlockout_verbose_trace=1

Allow the ability to set server debugging in the server configuration document

Currently, if you want to go in and debug a server, you must edit the notes.ini.  Or edit the notes.ini settings in the server configuration document to add the debugging lines.

 NOTES.INI should probably be promoted to being an NSF.  Doesn't have to be a complicated one, could be really simple:  each document has one, maybe two or three fields.  The main field would be plain text, another field would be an effective date field, another could be expiration field, and ...
